Last updated: November 5, 2026
This Privacy Policy explains how NuVitale (“NuVitale,” “we,” “us” or “our”) collects, uses and shares information when you visit nuvitalelife.com (the “Website”), contact us through it, or use the links on it to book an appointment.
1. This policy and your medical information
This policy covers the Website. It does not cover the medical care we give you. Health information we create or receive while treating you, such as your medical records, test results and billing information, is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). We handle it as described in our Notice of Privacy Practices, which you can request from our office at any time.
Please do not send medical details through the Website’s contact form or by email. Those channels are for general questions and appointment requests. If you need to share health information with us, call our office or tell your provider at your visit.
2. Information we collect
Information you give us. When you fill in our contact form, we collect your name, phone number, email address, the service you are interested in, and your message. We also collect any information you share when you call, text or email us.
Information collected automatically. When you use the Website, we and our technology partners collect information about your device and visit. This includes your IP address, browser and device type, the pages you view, how long you stay, the site or advertisement that brought you here, and cookie or advertising identifiers.
Phone call information. We use a call-tracking service that can show a different phone number depending on how you found us. When you call one of these numbers, the service records details such as your phone number, the time and length of the call, and which page or advertisement you came from.
3. Booking appointments
Our “Book Appointment” buttons take you to the third-party scheduling and patient portal services we use. Information you enter there is collected and stored by those providers on our behalf, under their own security and privacy terms, and becomes part of your patient record covered by our Notice of Privacy Practices.
